ABOUT THE OFFICE:

   The Thayer Group for Women’s Care is located at 1155 Alpine Avenue, Suite 230, Boulder , CO 80304   The office is on the North-West corner of Alpine and Broadway.  We are situated on the second floor, near the elevator, in the Medical Pavilion building.  It is connected with Boulder Community Hospital, Broadway Campus.  Free parking is available in our parking garage located directly across Alpine Ave. from the Medical Pavillion.  To find the easiest way to get to our office use www.mapquest.com or call our office at 303-443-2010.



Hours, Scheduling Appointments, Phone Calls: 

     Our office is open Monday through Friday and usually one Saturday each month.  There is someone at the office Monday through Thursday, 8 AM to 5 PM and Friday, 8 AM to 2 PM, to answer your call. 

     For your convenience there are days each week that we offer early morning and early evening appointments. We may be able to schedule an appointment for you the same day you call.  Let the receptionist know if this appeals to you.  We will do our best to accommodate your requests.

       When calling the office:

          -Let the receptionsit know if you need to speak to a nurse/medical assistant.

          -Let the receptionist know if your call for the medical assistant is urgent.

          -If you need a perscription refilled, please call the pharmacy first and have them  FAX a request to the office.

          -For test results or lab results ask to leave a message for the medical assistant.  This allows her to track your results and pull your chart.  If the matter is more urgent, be sure to explain that to the receptionist.

EMERGENCIES

    If you have an urgent medical need during business hours, please identify your need to the receptionist immediately when you call or arrive.  She will notify one of the medical personel directly.  If this occurs after hours, call the office number and you will be instructed on how to reach the doctor.

    There is 24/7 physician on-call coverage for true emegencies only.  If you believe you may have an emergency, please call and discuss your situation and symptoms with the doctor on call.  This will almost always be Dr. Thayer, but there will be times when another doctor is covering for him.  Please be prepared to identify yourself and explain the details of your situation, even if you are sure the doctor will remember everything about your last visit.  Dr. Thayer sees many people in any day, occassionally with the same name, so please forgive him if he asks you several questions.  If for any reason there is a delay in your receiving a call back from the doctor, understand that they may already be serving someone else in an emergency at the hospital or operating room and they cannot always get to the phone.  Do not delay in going to an emergency room or urgent care if you need assistance.  This emergency service is NOT to be used for making appointments or refilling a perscription
 

IF YOU HAVE A LIFE THREATENING EMERGENCY,  PROCEED TO THE NEAREST EMERGENCY ROOM OR CALL 911.  DO NOT WAIT FOR A CALL BACK!!! 



About Your Appointment at the Thayer Group for Women’s Care:


· Please arrive 15 to 20 minutes before your scheduled appointment.  There is invariably paperwork that has to be checked and updated.  If you do not arrive early understand that you will be subtracting precious time from your appointment with your caregiver.  They work hard to stay on time so as not to keep you waiting.  Don't expect them to delay the next person because of your tardiness. 

· If you are new to our office you will need extra time.  If you can please download the appropriate Complete Medical History form from this website and have it completed before you arrive, you will help prevent a delay.  It's a time consuming process to get your insurance info and fill out phone numbers and address etc.  If you have not completed your forms, allow plenty of time to do so when you arrive.  If you are relying on records arriving from another doctor, please check with us several days early.  You may have to call and remind them, based on our past experience.

· If you need to cancel or re-schedule an appointment please call the office at least 24 hours prior to your scheduled appointment.  As is true of so many other businesses we have had to adopt a cancellation policy.  There is a fifty dollar charge for not canceling with at least 24 hours notice or not showing up for an appointment . Insurance will not cover this charge.  Please have the courtesy to either keep your appointment or call us with enough advance warning that we may give your time slot to someone else.

· Payment for your care is expected at the time of service.

· We are happy to bill your insurance company for you.  Remember to always bring your insurance card with you and plan to pay your co-payment at the time of service.  We are required by the insurance companies to collect your co-pay at the time of service.
(There is a ten dollar billing fee if co-pay is not paid at the time of service.)

· If you are running late for your appointment, please call and let us know.  If you are more than ten minutes late for an appointment, we may need to reschedule your appointment.

-We are actually so happy you have chosen us for your care!!!  We believe you have made a very wise choice.  We hope that nothing about these instructions and reminders is going to discourage you.  We want to help you get the most out of your appointment.

Copying Medical Records:

     We are happy to provide you with a partial or complete copy of your medical records.  There will be a minimum of seven days turnaround.  Before your medical records can be copied and released we need your written consent.  This is to protect you and your confidential medical records.  The “Release of Records” form must be completed and signed by you.  There is a small fee for this service which is provided by an outside confidential vender.  You can obtain a medical release form online at Forms or from the office via fax, mail or in person. Remember we can copy records only with your written consent and prior notice.
